About Doxorubicin

Doxorubicin HCL is a cancer treatment administered through intravenous injection. Also known as hydroxydaunorubicin and hydroxydaunomycin, it is used in cancer management to treat various cancer types, including bladder cancer, Kaposi’s sarcoma, breast cancer, lymphoma, and acute lymphocytic leukemia. 

Regulatory Approval

Doxorubicin HCL was approved in the United States in 1974 and is on the World Health Organization’s List of Essential Medicines. In March 2022, the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) adopted a favorable opinion of the drug Zolsketil, a branded doxorubicin product. The statement recommended that the drug receive a marketing authorization to treat advanced ovarian cancer, progressive multiple myeloma, metastatic breast cancer, and AIDS-related Kaposi’s sarcoma.

Doxorubicin HCL is typically combined with other chemotherapy agents to achieve desired treatment results.

How Doxorubicin HCL Works

Doxorubicin interacts with DNA through intercalation (inserting or placing a molecule or compound between the base pairs of a DNA or RNA molecule) and inhibiting macromolecular biosynthesis. Both mechanisms hinder the activity of topoisomerase II – an enzyme responsible for relaxing supercoils in DNA during transcription. Doxorubicin stabilizes the topoisomerase II complex after the DNA chain is broken for replication, and this prevents the release of the DNA double helix, effectively stopping the replication process. Doxorubicin also enhances the production of quinone-type free radicals, which contributes to its cytotoxic effects.

The doxorubicin molecule contains a planar aromatic chromophore segment that inserts itself between two DNA base pairs. Concurrently, the six-membered daunosamine sugar portion occupies the minor groove and interacts with adjacent base pairs next to the intercalation site. Doxorubicin can induce the eviction of histones from transcriptionally active chromatin through its intercalation mechanism. As a result, cells exposed to doxorubicin experience deregulation in DNA epigenome, transcriptome, and damage response.

Anti-Cancer Activity

Research exploring the effects of doxorubicin HCL produced favorable endpoints, specifically combination therapy experiments using sirolimus to treat Akt-positive lymphomas in mice. 

Further, doxorubicin was combined with a murine monoclonal antibody in a 2006 animal study. This research led to the development of a specialized type of therapeutic agent designed to selectively target and deliver a cytotoxic drug (immunoconjugate) that eliminated HIV-1 infection in mice.

Potential Antimalarial Uses

Evidence from 2009 suggests that doxorubicin and related compounds may have antimalarial properties. The study found that a compound structurally similar to doxorubicin demonstrated the potential to inhibit plasmepsin II.

Plasmepsin II is an enzyme unique to the malarial parasite Plasmodium falciparum. Following the discovery, the pharmaceutical company GlaxoSmithKline identified doxorubicin as one of the compounds that has the potential to prevent malarial parasite growth. 

Potential as a Theranostic Agent

A theranostic (therapeutic and diagnostic) agent is a type of multifunctional compound or substance that combines both characteristics into a single product. 

Doxorubicin is known for its fluorescence characteristics, where certain substances can absorb light at a specific wavelength while emitting light at a longer wavelength. These properties have been used to assess doxorubicin’s potential as a theranostic agent, however the compound’s fluorescence spectrum depends on certain factors, such as the solvent dielectric constant and the environment’s pH.
